New Patient Forms

For new patients, please complete our New Patient Form before your first visit. If you have records with another dental office that you’d like transferred to our office, please also complete the Authorization Release Form. Bring these forms with you to your appointment so we can review your history and get started right away.

To make your visit even easier, you can complete these forms at home or arrive 15 minutes early to fill them out in the office. If you have any questions while filling out the forms, our friendly team is always happy to help.
